A: Legacy Health Services facilities are prepared to assist in admission from home or the hospital. Admissions to a nursing home are more common from a hospital because of further therapy or recuperation that may be needed after a hospital stay. Q: Who is eligible for Medicare?

A: Generally, Medicare is available to people age 65 or older, younger people with disabilities and those with End Stage Renal Disease. Medicare has two parts: If you have questions about Medicare, you can call 1-800-772-1213.

Q: How do I replace my Medicare Card?

A: Call the Social Security Administration’s toll-free number: 1-800-772-1213 and make sure to have your Medicare number ready when you call. You should receive your new card in about four weeks. You can also contact your local Social Security office.

Q: If I am receiving Medicare benefits, will Medicare automatically pay for my nursing home stay?

A: Not necessarily. Medicare has very specific guidelines that must be met in order for it to cover the cost of a nursing home stay. Depending on the patient’s condition and a qualifying 3-day hospitalization, Medicare can cover the cost of up to 100 days of nursing home care.

Q: Can I bring my own furniture and decoration from home to my new nursing home or assisted living room?

A: Yes. Legacy Health Services facilities invite residents to bring in their own pictures, small furniture and other personal belongings that will personalize their new home.

Q: Will my doctor follow me to the nursing home?

A: If your doctor is credentialed at a Legacy Health Services facility he/she can see you in our facility. If not, a family member can take you to see your doctor for required visits. Otherwise, the facility can make arrangements with qualified physicians who can begin caring for you at our facility.

Q: What is Medicaid?

A. Medicaid is a publicly-funded health insurance program that provides health coverage to those with low incomes, children, pregnant women and people who are aged, blind or who have disabilities.

Q: Who qualifies for Medicaid?

A: Adults 65 or older, individuals with disabilities and those that meet certain financial requirements. You will need a Social Security number, be a US citizen or qualified alien, a resident of Ohio and meet certain limits on income and assets.

Q: How can I apply for Medicaid in Ohio?

A: You can call the Ohio Consumer Hotline at 1-800-324-8680. The hotline is staffed from 8:00 a.m. to 8:00 p.m. weekdays and from noon to 5:00 p.m. on weekends. You can also call your local county’s Department of Jobs and family Services during weekdays.
