2022 Open Enrollment

Open Enrollment for 2022 benefits runs October 25- November 3, 2021.  A benefits representative will be on-site at each of our facilities to discuss benefit options for 2022.  Please note, for 2022, Sun Life will be the new provider for disability, life, accident and critical illness benefits previously offered through Lincoln.  Qualified Legacy employees working greater than 30 hours per week automatically receive company paid short term disability and life insurance.  Employees must call Sun Life to complete enrollment for these company paid products.

To complete enrollment with Sun Life, please call into the Sun Life Call Center anytime during open enrollment (M-F 9AM-6PM) 855-937-1955. Please be prepared to provide all
dependent data, including dates of birth and social security numbers.

Help us grow our team!  Do you know an RN or LPN that would be a good fit for our team?  Make an employee referral and you will earn $1,000 for EACH referred nurse who stays at least 90 days!  You do not have to be a nurse to qualify- any full or part time staff member can make a nurse employee referral (administrators, DONs and HR excluded).  Don’t delay, this program will only be active through the end of this year.  Please see your HR coordinator for more details!

OnShift Engage Winners

Congratulations to the following employees for being selected as our OnShift Engage prize winners! Through OnShift Engage, employees earn points throughout the year for completing feedback surveys, achieving tenure milestones and attendance performance.  Employees who reach the 250 or 500 point mark could choose from a variety of gift cards.  The employees with at least 750 reward points were given a raffle ticket for a chance to win our OnShift Engage annual drawing.
